Privacy notice

This notice describes how USA Insurance Policy Brokerage Inc. handles information submitted through this website and its client portal.

Information collected

The intake collects contact details, risk state, coverage type, current carrier, timing, preferred contact method and optional notes. Depending on coverage, it also asks for the insured’s legal name, risk address, business activities, estimated revenue and payroll, property characteristics, vehicle and driver summaries, requested limits and loss history. Unknown answers may be left blank.

Do not submit Social Security numbers, driver-license numbers, full dates of birth, payment-card or bank information, passwords, medical records, or other highly sensitive information through the general intake.

Why information is used

Information may be used to review an insurance request, identify missing underwriting facts, communicate about the request, prepare an authorized submission, maintain service records, protect the system, and meet legal or regulatory duties.

Sharing and market submissions

Information should be shared with insurers, wholesalers, program administrators, service providers, or regulators only when reasonably necessary and authorized. Submitting an intake does not authorize binding, changing, renewing, replacing, or cancelling coverage.

Security and retention

Administrative access is restricted, policy references are masked in the internal operating system, and documents are stored behind authenticated access. No security method is absolute. Records should be retained only as long as operational, contractual, legal, regulatory, or dispute-resolution needs require.

Client sign-in and portal privacy

Website requesters and approved clients may request a one-time code at their registered email address. The email provider processes the delivery address and code. Sign-in emails contain no policy information and have open and click tracking disabled.

We use essential, HTTP-only session cookies. Codes expire after 10 minutes; client sessions expire after 30 minutes of inactivity or eight hours. Temporary security records support retry limits and abuse prevention. Your guide completion or skip preference is saved to your account.

Account records and shared files may be available to other users the agency authorizes on that same account. Agency document uploads remain private unless explicitly shared. Internal document notes are excluded from client responses. Downloads remain on your device after sign-out.

Request confirmations and documents

After a website request is saved, we send a receipt to the submitted email address and a minimal alert to the agency. Our email service processes the address, request reference and message. These emails contain no insurance answers or attachments and have open and click tracking disabled. Verified requesters can view their own requests, add missing answers and upload private documents. Agency staff can review those records. Request access does not grant access to another account or its policies.

Optional saved drafts

If you choose Save for later, your unfinished answers are stored in this browser for up to seven days. They are removed after submission, on expiry when the form is next opened, or when you choose Delete saved draft. Anyone using this browser profile may be able to open the draft; avoid saving on a shared device. Consent must be given again when you submit.

Website measurement

We count page views, quote starts, saved drafts, submitted requests and phone-link clicks by broad source such as Google, Bing or direct visits. These counts do not include names, email addresses, form answers, full URLs, advertising identifiers or persistent visitor IDs. Browser interaction measurement respects Do Not Track and Global Privacy Control. Submitted requests remain part of agency service records. Temporary hashed network identifiers help limit abuse; counts are interactions, not unique people.

Choices and contact

People may ask to correct their contact or intake information, subject to legal and recordkeeping duties. Contact USA Insurance Policy at 347-684-0037 or info@usainsurancepolicy.com. Do not send sensitive identifiers by text, ordinary email, or voicemail.
